Care home or home Health, assessment

In most cases, your family or a home care agency can help you with household chores and personal care. If that is not possible, you may be able to get a spot in a nursing home. To do so, you need an assessment. An assessment is a decision that specifies what Health you need and whether you will receive them. You will receive this assessment from the Assessment Center for Long-Term Care ( centre Indicatiestelling) Health (CIZ).

You can check with the CIZ ( ) to see if you need an assessment for the Health you require. You can apply for the assessment yourself. However, you can also have a professional care provider submit the application on your behalf—for example, a home care agency or your family doctor. In urgent cases, a doctor must always submit the application.
