Nursing home or home care, eligibility assessment
In most cases, your family or a home care agency can help you with household chores and personal care. If that is not possible, you may be able to get a spot in a nursing home. To do so, you need a care assessment. A care assessment is a decision that specifies what care you need and whether you will receive it. You will receive this care assessment from the Care Assessment Center (CIZ).
On the CIZ website ( ), you can check whether you need a care assessment for the care you require. You can apply for the assessment yourself. However, you can also have a professional care provider submit the application on your behalf—for example, a home care agency or your family doctor. In urgent cases, a doctor always submits the application.
