Help with Applying for Health Care and Support
If you need help or support, you can receive free, independent client support. Someone who will review things with you and advise you on making difficult decisions.
What is independent client support?
A client support worker provides you with free information and advice on social services, (preventive) care, youth services, education, welfare, housing, employment, and income. The client support worker is independent and objective and therefore has no obligations to the municipality or any care provider.
Why Independent Client Support?
You may find it difficult to explain what kind of help you think you need. You may not have a clear picture of all the problems and possible solutions. Or you may need help making decisions. Ask for help—that way, you’ll avoid spending an unnecessarily long time searching for a solution.
Who is independent client support for?
Independent client support is available to all residents of Voorschoten who need help applying for care and support. People who are already receiving care and support, or who are providing informal care, can also make use of independent client support.
What an independent client advocate can do for you:
- Help you figure out exactly what your wishes and needs are;
- Helping people articulate those wishes and needs;
- Help people find the right information;
- Explain the application process;
- Preparing for meetings, such as the meeting with a Wmo consultant (also known as a “kitchen table meeting”);
- Accompanying them to appointments;
- Helping people make decisions;
- Help if you disagree with the care or support you are receiving;
- Work together with people around you to brainstorm and find solutions.
