Help with Applying for Health and Support
If you need help or support, you can receive free, independent client support. Someone who looks alongside you and advises you when making difficult decisions.
What is independent client support?
A client support worker provides you with free information and advice on social services, (preventive) Health, youth services, education, welfare, housing, employment, and income. The client support worker is independent and objective and therefore has no obligations to the municipality or any healthcare provider.
Why independent client support?
You may find it difficult to explain what help you think you need. You may not be able to oversee all the problems and potential solutions, or you may need help making choices. Request assistance to prevent spending an unnecessarily long time searching for a solution.
Who is independent client support for?
Independent client support is available to all residents of Voorschoten who need assistance applying for Health and support. People who are already receiving Health and support, or who provide informal care , can also make use of independent client support.
What an independent client support provider can do for you:
- Help figure out what your exact wishes and needs are;
- Help articulate those wishes and needs;
- Help search for the correct information;
- Explain how an application process works;
- Prepare for discussions, such as the conversation with a Wmo consultant (also known as a kitchen table conversation);
- Accompany you to meetings;
- Help you make choices;
- Help if you disagree with the " Health " or the support you are receiving;
- Think along with you and help find solutions with people in your network.
