Surname change

There is a difference between the product 'changing the use of a surname' and 'surname change'.

Changing the use of a surname applies, for example, when using your partner's surname.

  • If you wish to change your surname or that of your child, first check the Surname Change brochure by Justis to see if this is possible. Justis is the screening authority of the Ministry of Justice and Security.
  • If your situation is covered in this brochure, you can submit an application for a surname change to Justis.

What to do?

  • Download the application form and fill it out.
  • Check the Surname Change brochure to see which code applies to your situation. You must enter this code on the application form. You must enter this code on the application form.
  • Attach the requested documents to the form and send everything to Justis.
  • You will automatically be notified whether your surname will be changed. You will then receive an official extract of the Royal Decree.

Updating documents

  • If your surname has been officially changed, this must also be updated in your identity document. Therefore, apply for a new passport, identity card, and/or driving licence.
  • Health Also make sure to notify all relevant authorities of your name change. You do not need to notify the municipality separately; it will automatically receive a copy of the Royal Decree.

How long does it take?

You will receive notification from Justis within 20 weeks regarding whether your surname will be changed. This may take longer if your application is incomplete.

File an objection

If you disagree with the decision, you can lodge an objection with Justis within 6 weeks.
