Residents who get a flu shot are invited to vitality days

Because the municipality has difficulty reaching some of its residents, it sought to consult with the village’s general practitioners. Now that each practice is inviting approximately 800 residents to get a flu shot, this provides an opportunity to connect with this group of residents as well. After the vaccination, volunteers strike up a conversation with those who are interested and invite residents to the wellness afternoons.

Collaborating on Primary Care

“It’s wonderful that we’re working together on primary care in this way,” says Councilman Hubert Schokker. “If we want every resident of Voorschoten to be able to grow up, live, and grow old in good health, then we must also be able to reach every resident. By signing the Healthy and Active Living Agreement and establishing Samen Gezond Voorschoten, we’re well on our way. With this initiative, we’re once again putting our money where our mouth is.”

Space for two groups of 32 participants

During an hour-long conversation, staff members ask the resident how they’re doing, whether they’re familiar with the services offered by Voorschoten Voor Elkaar, if there’s anything else the staff member can help with, and so on. Finally, the resident is invited to take a fall risk assessment and receives personalized lifestyle advice. This pilot program has space for two groups of 32 participants. If successful, the initiative will be repeated at other locations.

The campaign is an initiative of Samen Gezond Voorschoten, the Plexxus physical therapy practice, the Alettahof activity center, the Voorzorg neighborhood partnership, Voorschoten Voor Elkaar, and the municipality of Voorschoten.
