VS-1247-1 Form for Reporting Changes to a Facility

Good to know:

If a hospitality or retail establishment undergoes a change that differs from what is specified in the issued license, the license holder
is required to report the change to the mayor within one month.

To ensure better and faster service, the municipality requests, at a minimum:

• Licensee information.
• Details of the most recent Alcohol License issued.
• Include floor plans of the location(s) and, if applicable, terrace(s), showing the current and desired layouts.
