VS-1134-1 Change of Manager for Alcohol License Using DigiD

Do you already have an alcohol license, and have there been any changes regarding the manager(s)? If so, please report these changes.

Good to know:

• You do not need to reapply for a permit to add a manager to the list.
• You can use this form to add one or more supervisors.
• You can also use this form to remove one or more supervisors.

To ensure better and faster service, the municipality requests, at a minimum:

• Licensee information.
• Details of the most recent Alcohol License issued.
• Has there been a previous amendment to the addendum to the Alcohol License? If so, when?
• Information about the new manager and the certifications and diplomas the manager holds.
• The manager's start date and whether he or she is a salaried employee.
