VS-1058-1 Statement from authorisation regarding an application for a travel document for a minor using DigiD

minor Children who wish to apply for a travel document need authorisation from the person(s) who has/have parental authority. 

authorisation is required in the following situations:

1. A child who is under 12 years of age when applying for an identity Map;
2. A child who is under 18 when applying for a passport;
3. An adult under legal guardianship

Who must sign? Person(s) with parental authority:

a. Both parents if they jointly exercise custody (Note! Also if the parents are divorced unless otherwise determined by the court);

b. Mother, when she alone is charged with parental custody;

c. Father, when he alone is charged with parental custody;

d. The authority that exercises custody;

Note! This form must be signed by all persons who are charged with custody.
