VS-1058-1 Declaration of Consent Regarding an Application for a Travel Document for a Minor Using DigiD

Minors who wish to apply for a travel document must obtain permission from the person(s) with parental authority. 

Consent is required in the following situations:

1. A child who is under 12 years of age at the time of applying for an ID card;
2. A child who is under 18 at the time of applying for a passport;
3. An adult who has been placed under guardianship

Who should sign? Authorized person(s):

a. Both parents if they exercise parental authority jointly (Note: This also applies if the parents are divorced, unless otherwise determined by the court);

b. The mother, when she is the sole holder of parental authority;

c. Father, when he alone has parental authority;

d. The authority exercising jurisdiction;
